Iris-Fixated PMMA Phakic IOL Explantation During Cataract Surgery

Beeran Meghpara, MD, discusses the case of a patient he observed for several years who had an iris-fixated PMMA phakic IOL, a progressively declining corneal endothelial cell density, and symptoms of cataract development. He describes his surgical approach to safely explanting the rigid, nonfoldable lens through a sutured superior scleral tunnel while protecting the compromised cornea. Cataract extraction was then performed through a separate temporal incision with a modified stop-and-chop technique.
