Posterior Tear

Soosan Jacob, MS, FRCS, DNB, manages a posterior capsule tear in a hard, subluxated traumatic cataract case. As the last fragments are removed under high suction, the floppy bag comes up and the phaco tip creates a punch hole. OVD is instilled prior to removing the phaco tip but vitreous still manages to prolapse through the incision. The author uses infusion through a pars plana trocar and the IOL scaffold technique to prevent the nuclear pieces from falling through the posterior capsule.
