An elderly man presents with a subluxated crystalline lens following blunt trauma. Zonular dehiscence progressed so quickly that by the time surgery occurred, less that 5% of zonules remained and the cataract was in the vitreous cavity. Capsule retractors are placed to stabilize the bag and a divide and conquer technique is used to remove the lens. Triamcinolone assisted anterior vitrectomy is performed and a Type 2L Cionni CTR is sclerally fixated via Hoffman pockets.
