Phoebe Lin, MD, PhD, and Sharon Fekrat, MD, FACS, present the case of a 51-year-old male with a traumatic dislocated crystalline lens associated with a superior 180 degree iridodialysis. This video illustrates a modification of the scleral-fixated IOL technique described by J. Prenner utilizing a single step for scleral tunnel creation and IOL haptic capture using a 25-gauge needle.

Very interesting! May i just ask this ... when you close the sclerostomy with the vicryl, do you include the prolene haptic in the suture bite? Thanks! karen

Is the IOL the same size of the IOL standard or is the especial IOL?

The IOL used in the video was a standard MA60AC IOL. The technique also works with other 3 piece IOLs. The majority of the haptic is fed into the tunnel. the sclerotomy through which it was externalized is closed in a water tight fashion with the vicryl suture, but no, we do not include the haptic in the suture knot itself, if that is your meaning, Karen. Thank you for watching!

excellent technique, most difficult part is to make these tunnels, I will try this technique in my next case. Thanks for sharing with us

Very interesting way of fixing the IOL Haptics in to the Scleral Tunnel. Will be of great help in such cases.Paras Mehta,INDIA

this seems like a nice way to make the side tunnel for the haptic which is one of the toughest parts. i do like to use fibrin glue as described by agarwal et al to close the flap. thanks for sharing your great case. tom o

Very clever! That looks like a nice improvement on the technique...I look forward to trying it! jp
