Uday Devgan, MD, presents an IOL exchange and iris repair that ensued from a complex cataract surgery. After viscodissecting around the lens, Dr. Devgan dials the IOL out of the bag. He leaves the old lens in the anterior chamber as he injects the new IOL. In this way, the new lens safeguards the posterior capsule as Dr. Devgan cuts and removes the old lens. Then, he uses a McCannel suture technique to reapproximate the iris.
