Pterygium Surgery: “New technique of bulbar flap conjunctival pedicle". This video shows a pterygium surgery with a new technique of bulbar flap conjunctival pedicle. The relapse rate in my cases is very low, attributed to the correct implementation of it . 1 - Apply lidocaine with epinephrine in pterygium. 2 - Apply duracaina lidocaine without epinephrine on subtenon. 3 – Take pterygium of cornea using muscle hook. 4 - Dry the pterygium. 5 - Dry the Tenon and edges of pterygium. 6 - Polish the corneal limbal with pterygium. 7 - Make a conjunctival flap without tenon for the cutting area. 8 - Cut Tenon flap area. 9 - Test that the conjunctiva isn´t too tight so that is doesn´t tear points. 10 – Do relaxed incisions. 11 -Place sutures to stabilize the flap on episclera. 12 – Place continuous sutures to secure. 13 – Apply triamcinolone to manage the inflammation. 14 - The key is to stick the flap in the immediate postoperative and not tear the sutures.
