John P. Berdahl, MD; Natalia Morales, MD; and Justin Schweitzer, OD, FAAO, review the case of a 78-year-old man with a history of mild glaucoma who now presents with visually significant cataracts. The patient reports nonadherence to his glaucoma drop regimen but has an IOP of 12 mm Hg. The panelists discuss how evaluation might differ for normal-tension versus primary open-angle glaucoma and consider the efficacy of MIGS procedures in this situation. They outline their decision to proceed with cataract surgery plus placement of a Hydrus Microstent (Alcon) and comment on next steps to pursue if the patient’s glaucoma progresses.
