Revision of an Ischemic Focal Bleb

Rachel G. Simpson, MD, and Roya Garakani, DO, describe a recent complicated case from the John A. Moran Eye Center at the University of Utah. The patient had a pre-existing ischemic focal bleb that was leaking despite the eye maintaining adequate pressure. Initial attempts to resolve the leak with topical medication and a bandage contact were ineffective, so surgery was performed to advance healthy conjunctival tissue surrounding the focal area of ischemia. After 2 weeks, the patient’s IOP spiked, and the bleb was highly inflamed, so Drs. Simpson and Garakani performed an additional revision.
