Rachel G. Simpson, MD, and Roya Garakani, DO, review a challenging case in which a patient with neovascular glaucoma and a complicated surgical history presented with highly elevated IOP and persistent pain. The patient had undergone prior tube surgery but experienced postoperative pain that could not be managed through noninvasive procedures. A tube exchange was performed, but the approach was unsuccessful, and the tube was later explanted. Drs. Simpson and Garakani review the next steps to managing this case, including endoscopic cyclophotocoagulation, and they comment on go-to approaches for managing chronic pain and inflammation.
