A 74 year old woman is referred by her primary ophthalmologist for evaluation of a reversed IOL optic. Seven months prior to the visit, the patient had cataract surgery OD with insertion of a one piece Tecnis MultiFocal Lens (TMF) (Abbot Medical Optics)– inadvertently placed upside down. In the accompanying video, The IOL inversion occurred shortly after proper lens insertion, during I/A of the residual viscoelastic. Before full unrolling of the IOL and prior to complete unfolding of the haptics, there is enough room in the capsule to allow for an inversion to take place. In this video, Tal Raviv, MD, shows both a two handed and one handed technique to restore proper orientation.

I demonstrated this technique as viewed in Video Journal Ophthalmology about 20 years ago with a 3 piece IOL. Hyperinflating the capsule bag with viscoelastic facilitates the flip and removes stress from the zonules. Kevin McAuliffe, M.D.
