Eric Rosenberg, DO, presents a 49-year-old man who had a penetrating globe injury 8-years earlier. He now has lenticular astigmatism secondary to IOL placement. In this IOL exchange, 5.5 mm wound is made to remove the anterior chamber IOL and leveraged to introduce a 3-piece IOL (MN60AC, Alcon). After the leading haptic was externalized and beaded, attention turned toward the more difficult to maneuver trailing haptic.
