Some who have had Refractive Lens Exchange (RLE) or Natural Lens Replacement are not happy with their vision, complaining of poor visual quality of severe halos affecting their ability to drive or even constant double vision. If "adaptation" has not occurred by 6 months, then it may well be the intraocular lens (IOL) that is the culprit and if this is the case then an IOL exchange may be required.
Some IOLs cause more problems than others, specifically ones that are in the family of "Zonal refractive" multifocal lenses. As those who sought RLE did so to eliminate or reduce their need for spectacles (including readers), exchanging the multifocal lens for a monofocal lens will be a major compromise.
At Centre for Sight, many patients with implant problems have had their lenses exchanged for an alternate multifocal lens implant - a trifocal diffractive lens.
This video shows the technique of IOL exchange used by Sheraz Daya, MD, FACP, FACS, at the Centre for Sight. A zonal refractive lens (M Plus) lens is removed and a diffractive Trifocal lens (Finevision) is implanted. [No sound]
