Conjunctivochalasis Treatment with Ellman Radiofrequency Surgery

This patient has marked conjunctivochalasis causing tearing and foreign body sensation that is treated using the Ellman Surgitron Dual RF 120 Generator under topical anesthesia in an office setting. Slit lamp video taken immediately before and after treatment demonstrates the reduction of the excess tissue and improved adherence of conjunctiva to the eye with lid movement.

I usually inject lidocaine with epi to balloon up the conjunctiva and then apply spots of cautery to the entire inferior conjunctiva. This is similar to the standard treatment for superior limbic keratoconjunctivitis. I use a bullnose tip on my Ellman set for cautery at 3-4 on the dial. I then patch the patient overnight and have them use tobradex ointment TID for a week.
