20-Gauge Vitrectomy Under Temporary Keratoprosthesis for Shattered Globe Reconstruction

A ruptured globe with central corneal laceration and vitreous hemorrhage presents a surgical challenge. When managing such complex trauma cases in which the cornea does not allow a good view of posterior segment, surgeons should consider vitrectomy under a temporary keratoprosthesis. Temporary keratoprosthesis is placed by the cornea team and replaced by a permanent donor cornea after the retina team has restored the posterior segment anatomy. The technique allows excellent visualization of the posterior segment during vitrectomy in cases of traumatic corneal damage.

Thank you for youe comment. Boston Keratoprosthesis type 1 was used in this case.

Sorry - the previous response was for the permanent K-Pros we generally use, however in this case we used a corneal transplant at the end. I apologize for the confusion. For temporary KPros we use Ekhardt's non-flexible PMMA temporary KPro. In this case we used the 2.5 mm KPro which is designated for aphakic eyes.
