This is a complicated case of a patient who had UGH syndrome for months prior to consultation with a 2.5 mm pupil. This video shows the steps needed to exchange a rigid PMMA lens without the use of iris hooks. This case also highlights the use of a 4 point fixated gortex lens. Lastly, I utilized the ECP probe for something not many use it for: cautery. I felt the need to see endoscopically under the iris to visualize the bleeding areas and I cauterized all areas of concern. Since this case, the patients vision has improved and her glaucoma and recurrent vitreous hemorrhages are resolved.
