Transconjunctival sutureless vitrectomy has undergone a major transition with the introduction of small gauge vitrectory instruments. The use of trocar cannula systems have revolutionized the approach to maintaining globe infusion and has become a fundamental component of posterior segment surgery. This video describes an improvised technique for trocar anterior chamber maintainer (TACM) where a trocar cannula is introduced into the limbus to maintain adequate infusion in the eye.
