Toric IOL Gone LRI

A patient with a visually significant cataract, prior vitrectomy, and a high degree of against-the-rule astigmatism was undergoing laser cataract surgery with insertion of a toric IOL. However, during nuclear disassembly, a posterior capsular tear was noted due to excessive manipulation of the lens nucleus. The lens nucleus and cortex were successfully removed and a manual limbal relaxing incision was created to correct the patient's astigmatism. Placement of a three-piece IOL in the sulcus was successfully performed and the patient was 20/25 uncorrected despite an unintended intraoperative quicksand moment.
