In this case, an aphakic patient underwent cataract extraction with no IOL implantation 25 years ago, and desired spectacle independence. Ozana Moraru, MD, proposed secondary PC-IOL implantation. Intraoperatively, Dr. Moraru observed that the posterior capsule was absent and presented irregular cortex remnants and a Soemmering ring. Thus, she chose to implant an Akreos IOL (Bausch + Lomb) sutured at the sclera in four points using Gore-Tex sutures. Before implanting the IOL, she managed the large and thick Soemmering remnants. The video presents the technique for its removal by anterior approach only, followed by the IOL implantation.
