A 39-year old male presents with a dense white cataract that began developing after a bicycle accident more than 20 years prior. Trypan blue is used to stain the anterior capsule. Barry Schechter, MD, performed phaco at 100% power (Stellaris, Bausch + Lomb) to break up the dense material. OVD was continuously replaced while the nucleus was disassembled out of the bag. Unexpectedly, calcified lens fragments resisted emulsification and ricocheted off the phaco tip. After manually removing the calcified pieces, a Bausch + Lomb aspheric posterior chamber lens was easily placed in the bag.

ECCE was considered, but as mentioned, the patient wished to return to physical activity as soon as possible and phaco was agreed upon. As to the background music, it matched the patient's upbeat attitude and hopefulness of a positive result which he did not get from his ophthalmologist in Haiti.

I would like to commend Dr Schechter for sharing video of his surgical case that helps reminding all of us how post-traumatic white cataracts do develop an extremely dense nucleous, “masked” underneath a dense white coat, if left there for decades. In the case shown, Dr Schechter's choice of dislocating the lens in the AC is reasonable. The choice of employing phacoemulsification technique instead of ECCE procedure also brings unquestionable advantages. My only concerns regard the phaco-technique adopted: if it true that it’s always best to use technique and instruments we are mostly familial with, horizontal phaco-chop technique would have reduced the amount of energy dispersed in the eye. I very much agree with Nick that this is the "Hardest" cataract only in Florida. We in the thirdworld see this kind of Cataracts quite regularly. In my opinion the use of Phaco energy in this case was a kind of over kill and an Planned ECCE from the very begining should have been considered.

Nice Surgery! Small incision ECCE, as practiced in India and other areas of the world where this type of cataract is the rule, rather than the exception, may have been easier. Unfortunately, this is a lost art in the US, as most residents as trained now have never even SEEN an ECCE, let alone done one.

I agree with ZMario, the music is a distraction. But more important: every day in poor regions of the world, surgeons have to deal with cataracts like this. They've no phaco, no Vision Blue, none of the expensive pharma and accessories which are taken for granted in a Florida eye centre. So, is it the "World's Hardest Cataract" - and how does one say that with certainty? Is it not more likely to be just the hardest cataract in that part of the world, which is Florida? Last questions: why would a 39 year-old wait 20 years before getting treatment for this - whether it be in Florida, Haiti or anywhere? Why was phaco considered the best option?
