Shannon Wong, MD, presents suture fixation of a dislocated/subluxed Tecnis Symfony IOL (Johnson and Johnson). The original surgery on 9/14/16 was routine and uncomplicated. The patient was seen on 10/25/17 (more than one year postop) with normal vision and IOL contraption. Then, on 11/3/17, patient presented with decreased vision and no history of trauma. Gore-tex sutures were used to sclerally fixate the IOL in-the-bag complex. Within one day after surgery, the patient's vision returned to baseline normal.

Ari, If my patient had a superior bleb, I would have used the identical technique as I did not disturb the superior conjunctiva.

i have a case just like this, but she has a bleb superiorly. she is s/p air bag injury and the inf haptic is sitting on the inf pupil. any thoughts on what i should do?

The lack of fibrosis makes me think of a dead bag (or partially dead as there is some fibrosis superiorly. Did you consider an Ahmed CTS? This would have avoided piercing the bag and made adding a CTR more comfortable. Good to be lucky and talented. Congratulations on a nice repair
