This video presents a technique of scleral IOL suspension (Akreos Adapt AO; Bausch + Lomb). A Gore-Tex suture is passed transconjunctivally and the IOL is inserted through a 2- and 2.4-mm corneal incision using the IOL injector. Two cases are presented: the correction of a posttraumatic aphakic eye and the correction of a subluxated bag-iol complex, in a patient with pseudoexfoliation. In both cases, a minimally invasive scleral IOL suspension was performed, achieving fast visual recovery and a functional result of 20/25 and 20/20, respectively, with a residual refractive error of less than 0.75 spherical equivalent. In conclusion, this technique allows for scleral iol suspension procedures to be performed as minimally invasive. This results in less aggression, enhances visual recovery, and improves predictability.
