The video presents a case of a spontaneous dislocation of single-piece IOL, which is mobile in the anterior vitreous cavity with the inferior haptic hanging on the pupil in front of the iris. The eye has extensive iris stromal atrophies and a fibrotic rigid pupil, with maximum dilatation of 3 mm. Ozana Moraru, MD, removed the dislocated IOL by manual elevation in front of the iris, through the small rigid pupil, with the IOL cut in two halves and subsequently removed through a 3.8-mm limbal incision. A thorough vitrectomy, including the checking of the peripheral retina, followed. An Akreos IOL (Bausch + Lomb) was sutured at sclera in four points using Gore-Tex sutures.
