The video presents a case of a highly myopic patient who was implanted with an Artisan refractive phakic lens 20 years ago, and now presents with significant visual loss (hand movement) due to a mature cataract. The surgery starts with the removal of the PMMA Artisan lens through a scleral tunnel incision under dispersive and cohesive OVD protection, followed by phacoemulsification through a new limbal incision. Before Artisan removal, because the white cataract needed a stained anterior capsule, Dr. Moraru injected the dye not only before the Artisan removal, but also before the OVD injection. Then, the Artisan was carefully removed, followed by the standard steps of the phacoemulsification, with posterior chamber IOL implantation.
