A 59-year-old man presented with reduced vision and fundus examination showed a retinal detachment involving the macula. Andre Maia, MD, and Renato Rosario, MD, decided to perform a combined phacoemulsification lens removal with PPV for macula off rhegmatogenous retinal detachment. Even with a 27 gauge light source, visualization with the Ngenuity (Alcon) is quite good.
