A 63-year-old patient with a longstanding history of diabetes mellitus presented with a moderate cataract and progressive visual decline, reduced to count fingers at the time of surgery. Despite prior treatment—including panretinal photocoagulation and three bilateral intravitreal injections administered 3 years earlier—the disease had continued to progress, as evidenced by fundus photography, fluorescein angiography (FA), and optical coherence tomography (OCT).
