This is a case of an insertion of a piggyback implant. A STAAR AQ5010 on top of a Trulign. The patient had a diopter of a myopic refractive surprise and wanted this corrected. The solution that the patient chose was to have the AQ5010 inserted piggyback on top of the previously inserted Trulign.

If I had had a +1.00 D refractive surprise (instead of a -1.00 D), I would have handled it the same way with a piggy back AQ5010 IOL in the sulcus.

would you have handled a refractive surprise of +1.00 the same way, with a -100 sulcus IOL?

The original surgery with the Trulign insertion was routine without any complications. There was no forward tilting or vaulting of the IOL, both haptics were in the bag with a well centered and ? 5.5 mm capsulorhexis. I do not have access to an ORA at this time. Post-operatively, the cylinder was 100% corrected with the Trulign; there was a 1D myopic refractive surprise most probably secondary to her ELP. I cannot explain the refractive surprise any other way.

One month after the AQ5010 piggy back IOL insertion over the Trulign, the visual acuity in the patient's operated eye was as follows: UCDVA: 20/25-1 UCIVA: 20/25 UCNVA: 20/40 I have found that the Piggy back IOL does not effect the Intermediate and functional near vision over Crystalens or Trulign IOLs.

you gotta figure that the minimal anterior haptic movement of the trulign will be reduced to zero once the piggyback is in the sulcus
