In this case, a 56-year-old man presented with a mature cataract and fibrotic capsule in his left eye. The other eye had an NS +2 cataract. Microscissors were needed to perform the capsulorhexis, but the fibrotic nature prevented posterior extension. The leathery cataract required phaco chop to be performed to the posterior plate. Eventually, a toric enhanced monofocal PCIOL was positioned in the bag.
