Devesh K Varma, MD, presents a case of a dense lens in the presence of weak zonules and a shallow anterior chamber. A CTR is placed prior to beginning phacoemulsification with a debulking technique but it is quickly apparent that lens density is adding tension to the zonules. Capsular Support Hooks (MST) are placed to stabilize the capsule during the remainder of phaco.

I tore the anterior capsule when using CSH during a case. Do they work better if you insert them at a steep angle so the hook is angled back up into the bag? Or making the incisions parallel to the iris plane?

It's not very safety. The capsule maybe torn with the CSH. The CIONI ring is better.
