Pediatric IOL with PPV

Carol Drake, MD, presents a challenging case of a pediatric cataract in a three month old infant. Creation of the capsulorhexis was so difficult that the surgeon had to utilize a vitrector to complete the capsulotomy. A one piece IOL (SN60 Wavefront, Alcon) is inserted through a scleral incision and maneuvered into the bag. A 23-gauge Pars Plicata vitrectomy and posterior capsulectomy are performed.

I really don't know. Perhaps I didn't put enough in, didn't leave it in long enough. The cataract was unusual as there was liquid cortex leaking out after the initial opening of the capsule (for a pediatric cataract). It didn't tear well either although the edge remained strong during insertion.

I used to do it that way but as I tried to put lenses in smaller and smaller eyes I found it really difficult to make sure the lens was in the bag. In this eye if the posterior capsule was open there would have been no way I could have used the force I did to get that lens in the bag without it getting pushed back into the vitreous. Doing a PPV has made my life so much better and allowed me to put lenses in even smaller eyes.

why did vision blue not work? i have never seen it fail in any case i have seen or done.

why not do capsulotomy and anterior vitrectomy via limbal incisions prior to lens placement? that would reduce posterior pressure. the sn60 looks so huge in the tiny eye!
