IOL exchange using 'Pacman' IOL cutting technique. In this case, the patient had persistent photopsias with an Acrysof acrylic toric IOL. Limbal relaxing incisions were placed to account for the corneal astigmatism, then the IOL was removed by cutting a quadrant out of the optic (creating a 'pacman' shape) and rotating the rest out of the 3 mm incision. A Tecnis acrylic monofocal IOL was inserted in the capsular bag, and postoperatively the patient's photopsias resolved.

Great technique, one variation that I can suggest is to extend the first cut on the IOL approximately 60% of the IOL , then externalize one of the haptics and pull and rotate the IOL around the wound.

A photic phenomenon that can blur the diagnosis in the setting of flashes of light in a normal-appearing exam include dysphotopsia from intraocular lens (IOL) placement after cataract surgery, termed pseudophakia dysphotopsia. Patients experience flashes, streaks, or reflections emanating from a central point of light that has been reported to spontaneously resolve in two-thirds of patients. These lights are due to illusions and not true phosphenes. Symptoms can be severe enough to warrant IOL exchange and may be related to artificial lens material or shape

ive read that the culprit for photopsia is overlap of the ccc on the edge of iol. so some srgeons do optic capture, often with 3 piece iol, to get the ccc behind the optic.
