The first pseudoexfoliation case presents the typical signs of very loose zonules. After making the capsulorhexis with forceps, multi-direction hydrodissection helps mobilize the lens. Lisa Arbisser, MD, employs a chop technique and carefully supports the chamber will BSS. Dr. Arbisser delivers cortical material slowly under low-vacuum. After the bag is clean, a CTR is placed in the bag and an IOL is placed in the sulcus. As Dr. Arbisser explains, a three-piece lens with its haptics in the sulcus and its optic captured through the CCC to tamponade capsular contraction.

I would congratulate dr Arbisser for employing this technique. I think it's a really effective technique and represents my first choice of IOL implantation in any case of zonular weakness. It's very useful injecting the three-pieces lens instead than implating with forceps, as shown. The manipulation of the IOL seemed to be a bit excessive, particularly in light of the diffuse zonular instability due to the PXF syndrome. I also feel that in those cases the bimanual I/A should be preferred to the coaxial I/A. I would like to encourage everyone to become familial with this technique and consider it as a first option of IOL implantation in any case of zonular instability. I would also like to thank dr Arbisser for showing us her surgical technique.
