This patient presented to us with a dense white cataract with a past history of trabeculectomy done for chronic narrow angle glaucoma 4 years ago. The patient had a very shallow ac of 1.7 mm and a non-dilating pupil. The procedure was challenging, and this video shows how each and every challenge was managed.

I would have used a 23 guage valved trocar system port through pars plana and done a minimal dry vitrectomy. Lens -iris diaphragm will move posteriorly enough to safely proceed
