Morgagnian Cataract with Femtolaser Assisted Surgical Complication

This hyperopic, sick eye with Morgagnian cataract, despite adjustment of femtolaser parameters of energy and fit, suffered a grossly incomplete capsulotomy. Because of lens milk clouding the view of anterior segment landmarks and spontaneous bleeding from the iris root associated with occult neovascularization a conversion to extracapsular technique was used to remove the nucleus. Ultimately the capsular remnants were removed, a limbal anterior vitrectomy was performed and the small eye left aphakic due to poor visual prognosis.
