Monofocal Piggyback Sulcoflex IOL in Pediatric Case

William Astle, MD, FRCSC, discusses the management of a pediatric patient with a persistent hyperplastic primary vitreous (PHPV) type cataract. The patient underwent surgery at 8-weeks with an IOL implant targeting +8 diopters. In just over three years the patient's vision had a 15 diopter shift to -7 diopters and was developing a secondary membrane. At this time the surgeon opted for a monofocal piggyback Sulcoflex IOL (Rayner) implantation and removed the membrane from the visual axis with a cutter.

This lens looks like a one piece with haptics that do not belong in the sulcus due to iris chafing???
