Blunt trauma to the eye resulted in cataract, iris sphincter microtears, and extensive zonular dialysis. Triamcinolone is injected into the anterior chamber in order to visualize the vitreous, then the bag is filled with dispersive OVD to displace vitreous strands posteriorly. A Malyugin modified-CTR (Morcher), which is designed to center the subluxed lens capsule by scleral wall fixation, is injected under the anterior capsule in the direction of zonular defect. Once the ring has been centered on the area of zonular weakness it is sutured to the scleral wall. Boris Malyugin, MD, then uses the Stellaris phacoemulsification system (Bausch + Lomb) to disassemble the nucleus with a quick chop technique. After an Akreos intraocular lens, a bimanual anterior vitrectomy is performed.

Why not do anterior vitrectomy at the beginning when you have stained it with triamcinolone?? It would prevent it prolapsing forward during phaco and IA. You are lucky to have had no problems with this modified CTR but I would use a Ahmed capsular tension segment and suture that in the area of zonular dehiscence. It would give me less of a headache than seeing the CTR damaging more zonules. Capsulorhexis was very well accomplished. Congratulations on a successful outcome.

I agree with Dr Shalabh, definitely a case to use CTS and a standard CTR.

not clear what is done with the suture once the two passes through the sclera are made?
