Vikas Shankar, MD, Muhammad Amer Awan, MD, and William Wykes, MD, describe a technique to explant a posterior chamber intraocular lens (IOL) without the need to enlarge the incision or cut the haptic and optic. The technique involves injecting Healon under the IOL to inflate the bag and prolapse IOL into anterior chamber (AC). Sinskey hook supports the IOL from behind and a lens insertion forceps is used to fold the lens in the AC, keeping parallel to the sinskey hook. Both the instruments utilise the same incision and the folded IOL is explanted without need to enlarge the incision. It is a safe and effective technique which can be used to exchange IOL in the situations like damaged IOL and refractive surprises.

How do the three piece acrylic lenses handle? For example the MA60AC. Or silicone lenses. Are those too slippery? Thanks for the video

Yes you can use an incision 180 degrees opposite to the main wound. However I would recommend the main incision for Sinski's hook as it will provide a better control to fold the IOL as both hands will coordinate better if used from one incision and there will be no extra incision.
