Intrascleral Posterior Chamber Lens Implant Fixation Combined With 23-gauge Plana Plana Vitrectomy: Temporary Haptic-fixation with Silicone Plug

Mahi Muqit, PhD, FRCOphth; David Mordant, MD; and C.K. Patel, FRCOphth; describe a modification of the intrascleral IOL fixation technique using a temporary silicone plug fixation of the exteriorized leading haptic following 23-gauge pars plana vitrectomy surgery. During IOL optic centration adjustments, the leading haptic remained anchored by the silicone plug under the scleral flap. Once the IOL position was correct, the silicone plug was removed, and the leading haptic tucked into the intrascleral tunnel. The learning curve for this technique is minimal for surgeons familiar with manipulating iris hooks. The technique is safe, achieves effective haptic anchorage, and in addition, allows adjustable IOL maneuvers to achieve optimal centration during intrascleral IOL haptic fixation. We propose this technique for both anterior segment and vitreoretinal surgeons.
