Following a severe corneo-scleral laceration which was managed three months earlier. This gentleman presented with an extensive corneal scar, aphakia, aniridia and a vitreous hemorrhage. Vitrectomy through a temporary keratoprosthesis followed by an open sky IOL scleral fixation and a penetrating keratoplasty were performed. What was discovered in the middle of the surgery was unexpected and necessitated strong nerves and a steady hand.
