En Bloc Double Peel of Epiretinal Fibrovascular Membrane and ILM

Show Description +

This video shows a great vitrectomy with double peel where a taut fibrovascular epiretinal tissue was sitting on the macula and optic disc area, increasing retinal traction and edema from diabetic retinopathy. The ERM and ILM were both removed and endolaser was performed. The patient will benefit for antiangiogenics but the main traction from mechanical fibrovascular tissue pulling on the retina as well as distortions caused by the ILM were removed.

Thanks for asking, MI S. The laser used during this surgery is from the Alcon Constellation System, which is in the vitrectomy set. It is a class IV laser. Its wavelength is 532 nanometers (visible threshold). The reason you don't see it, since you could see the greenish beam from outside the microscope is that between the ocular lenses and the laser light that comes back from the retina towards the ocular lens there is a filter, which blocks the laser light with a twofold reason: protecting your eyes from this laser light and protecting the recording camera lens, which is immediately above the laser filter, otherwise it'd get its lenses and its pixel system damaged. If I don't have the filter "on", I'll get this light back into my retina and it is not desirable. Plus, I'd get my camera damaged.

what is the type of endolaser? It does not shinning the eyes at all?
