Learn how to eliminate eye drops from your cataract post-operative routine. Based on the work of Gills et al. and refined by Galloway and Liegner, this short video demonstrates injection of TriMoxiVanc to the vitreous cavity without breaking the capsule or utilizing a pars plans approach. This ophthalmic compound contains a stable solution of moxifloxacin, vancomycin and triamcinolone. A tuberculin syringe and a 27-gauge cannula are used to instill .2cc of medication. Experience with this compound now exceeds 14,000 procedures. Less than 10% of patients requiring steroid rescue at some point in their post-operative management.

Very interesring. The cannula must cross trought the zonules?

I tried it in uveitic cataracts years ago and it is not trivial to get the cannula in the right place. I see absolutely no indication to disturb the vitreous in normal cataracts. These substances (1 mg, 0.025 ml) can be injected in the anterior chamber with no risk for glaucoma and triamcinolone will hardly be visible next day. Much easier and equally effective.

lol! count me out! less than 10% steroid rescue probably means 9%, which is just too much for my taste. i will wait for a better method to come around. i certainly love the idea of avoiding drops, however.

Absolutely. When the 99% method shows up, count me in. In the meantime, 9 out of 10 think I am the greatest, the other one, thinks I'm just as good as the guy next door. Visit me on a postop day, and watch the smiling faces! Try it, you'll like it.
