Robert J. Weinstock, MD, presents the case of a dense cataract with a fibrotic membrane on the capsule. Bimanual phaco is used to sculpt the 4+ nucleus. During fragment removal, a hole in the posterior capsule is noted. However, with bimanual phaco the irrigation remains superiorly, which helps to keep the lens fragments high in the anterior capsule and in the bag.

Nice work. I had to put timoptic on my tongue to keep my bp down just to watch this...
