Samuel Masket, MD, performs cataract surgery in a previously vitrectomized eye for macular hole. During hydrodissection the globe became firm, the chamber shallowed, the OVD escaped and there was iris prolapse. The cause was fluid misdirection resulting from a small hole in the posterior capsule made at the time of the vitrectomy.
