Capsule Cleansing of the posterior and undersurface of the anterior capsule is demonstrated. The central posterior capsule is polished with a silicone tipped cannula on a 5cc saline filled syringe. The undersurface of the anterior capsule is scrubbed with a Rentsch Capsule Curette (Geuder) for most of the capsule and a Singer Sweep (Epsilon) for the subincisional area. The case is concluded with IOL placement and evacuation of OVD with liberated lens epithelial cells.

EXCELLENT VIDEO..... i prefer to keep the anterior capsule epi. cells left a little for early fibrosis to occur in torics

I'd ask you why you did't use the second capsule end of the double ended interument from EPSILON. It can easely sweep the other meridien by entering the main incision???

I find the Rentsch or Shepard curette to be more efficient for the purpose of cleaning LECs. The Singer Sweep though allows access to the subincisional area which the other devices do not. The opposite end of the Sweep does work through the main incision but leaves a gap between the main and paracentesis incisions. The combination of the two devices works best for me.

A toric lens was implanted. Would it be better to leave the lens epithelial cells on the anterior capsule to ensure early fusion of the capsular bag and prevent rotation of the toric IOL?

I've gone through an evolution of capsular polishing with respect to toric IOLs. I always polished the central posterior capsule and directly remove OVD behind the lens. I started polishing anterior capsule LECs across from the incision with IA and had a couple of rotations, so I then stopped this element. No rotations but more YAGs. I then began using the Rentsch curette and finally the Singer Sweep on non-toric cases and observed those IOLs for rotation before using this technique with Toric IOLs. No rotations. I understand the rationale of not polishing/curetting but my experience with cleaning 360 degrees of the bag goes against this. If I get a rotation I'll post it.
