Ozana Moraru, MD, presents a case of a patient with long-standing pseudophakia after traumatic cataract. A PMMA IOL is caught between the white fibrotic capsular bag, with a YAG capsulotomy in the center and a thick Soemmering ring in the periphery and extended iris posterior synechiae. The pupil also presented with superior corectopia. The patient was disturbed by the limited visual acuity, but especially about the white appearance of the pupil. Dr. Moraru presents the maneuvers to remove the IOL, the white fibrotic capsules, and the thick Soemmering ring, followed by the implantation of a foldable IOL in the sulcus.
