A dense cataract with a history of blunt trauma has poor zonular support and a Diamatrix XpandNT pupil expander is used to provide capsular support during phacoemulsification. The capsule is fixated to the sclera using two Ahmed segments so that a toric lens can be placed in the capsular bag. 5-0 prolene sutures are used to fixate the Ahmed segments to sclera with a flange technique.
