Two cases of 3D Sub 1mm MICS Surgery

In the first case, Sub 1mm MICS Surgery is performed in a grade 3 cataract using the Stellaris MICS platform. Jorge Alio, MD, makes sub-1mm incisions 90 degrees apart. After removing the lens quadrants, cortex is aspirated with bimanuality which enables the surgeon to access almost every part of the capsular bag. The second case includes astigmatism correction with limbal relaxing incisions. One LRI is opened to approximately 1.8 mm to accommodate an Akreos MI60 IOL (Bausch + Lomb). At the end of the case, the wounds are hydrated.

Dear Colleague,<br />Thank you Shalabh for your interest in our technique. We do not dilate the pupil preop with topical medication to preserve the ocular surface. We have not used topical midriatics for cataract surgery for the last 5 years, in which intraocular midriatics was used. With only one intraocular injection of 0.1 cc of Cyclopentolate 1% plus Fenilefrin 10% and Lidocaine 1%, all preservative free, prepared in our pharmacy at our institution, we dilate every case for cataract surgery. This has the advantage of transforming a manoeuvre that is usually perfomed by nurses into a surgical manoeuvre controlled by the surgeon. <br />The Akreos is a very good IOL, and you should expect a slightly higher rate of PCO as happens with all hydrophilic IOLs. This has not been a disadvantage in my hands. I use it because I like the neutral spherical aberration concept rather than the induction on purpose of intraocular aberrations.<br />Good luck!<br />Kind regards,<br />Jorge Alio <br />

Why don't you dilate the pupil pre-op? Any particular reason? I have seen more PCO with the Acreos than other aspheric IOLs. What is your opinion?
