Hello everyone. I'm Matteo Mario Carlà. I come from Italy and I work in Paris in the Rothschild Foundation and the French Institute of Myopia. My talk in this Euretina Congress was about the longitudinal evolution of myopic maculopathy in the European cohort. It firstly focused on analyzing the evolution of atrophic and tractional and neovascular maculopathy in highly myopic patients. Data were collected in Barcelona gathering of over 1000 patients from 2008 to 2024. And we found out that almost 57% of patients progressed in different forms, particularly patient which has been at baseline had tested fundus atrophy didn't progress as much. Only 1% of them developed macular atrophy at the end of the study. Otherwise, patient with diffuse chorioretinal atrophy and patch atrophy showed the higher rate of progression, which was associated also with the higher visual loss. In particular, patient with patch atrophy at the nine-time risk to develop macular atrophy at the end of the study.
In particular, also the shape of the eye was an important contributor in developing macular atrophy. In particular, compound staphyloma like type-nine staphyloma were associated with the higher rates of atrophy in the foveal area. While type one and type two, we analyzed the role of the choroidal thickness in macular atrophy development. And we found out that the baseline choroid thickness below 40 microns was an independent factor of risk to develop macular atrophy at the end of the study and to develop macular loss. Otherwise, dome-shaped macula was associated with development of myopic neovascularization at the end of the study and with higher degrees of choroidal thickness, which was a good evolution in this kind of patient. Finally, we focused our analysis on perivascular patch atrophy, which is a finding typical of patient with patch atrophy in more than 60% of cases. And also the patches around the vascular archaic showed an evolution in 90% of case.
And in particular, they tended to coalesce in around 30% of cases leading to macular atrophy risk four times more than patient without this kind of aspect. So overall, our study focused on the evolution, natural evolution of myopic maculopathy showing that this pathology is progressing and the most of focus we have to put is on eye shape. So the localization of atrophy, the eye shapes interval saphenoma and the development of other complications such as neovascularization. Thank you very much.
