This video demonstrates the management of a routine white intumescent cataract. During phacoemulsification, a large piece of the nucleus was dropped into the anterior vitreous. Ahmed Assaf, MD, utilized a scaffold technique by implanting a three-piece IOL under the fragment followed by an ample amount of dispersive OVD to protect the corneal endothelium. This was followed by phacoemulsification of the remaining fragment and alternating pars plana and limbal anterior vitrectomy for the remaining cortex and vitreous. This centralized the IOL and rounded the pupil after surgery.
