Stephen G. Safran, MD, presents a case of a 14-year-old patient referred with bilateral ectopia lentis. Dr. Safran's plan was to place a Toric Symfony IOL (Johnson & Johnson Vision) which is done successfully using capsule retractors and a Gore-Tex-sutured modified CTR to secure the capsular bag, but a decision to expand the rhexis opening at the end of the case led to a self-inflicted nightmare that needed to be fixed.

Great job I think after CTR insertion, any bag manipulation carries a risk for anterior or posterior tears to expand
