Ozcan Kayikcioglu, MD, presents a patient with proliferative diabetic retinopathy that undergoes a 23-gauge pars plana vitrectomy and membranectomy. A sharp instrument comes in contact with the sclerotic lens, necessitating a lens removal. The nucleus drops in early stages of phacoemulsification. Since the cannulas are already in place, the surgeon is able to remove the lens with a cutter. Endolaser is performed to treat an iatrogenic retinal tear and an IOL is placed in the ciliary sulcus.

Although we did not have any trouble in this case, adviced IOL in such cases is three piece IOLs for sulcus implantation.

looks like an alcon sn60wf-type iol. may cause trouble in the sulcus (ugh syndrome/iris chafing)
